We advise families in matching resources with the needs of an elder or chronically ill adult. We provide guidance and recommend options for some or all of the following services:
- Care Management Services for Elders and Adults with Chronic or Acute Illnesses including evaluating situation, identifying care needs, and providing home care, hospice, housing, and professional service options and referrals.
- Caregiver Support and Social Services including counseling, support groups and education for those providing the care for elders and those with chronic and acute illnesses as well as coordination of social support services to provide assistance and relief to caregivers.
